ENFERMEDAD ATEROSCLEROTICA RENAL. NEFROPATIA ISQUÉMICA: PRESENTE Y FUTURO.

En las últimas cinco décadas, la patología cardiovascular (cardiopatía coronaria, enfermedad cerebro-vascular y enfermedad vascular periférica) ha tenido un papel preponderante en la morbi-mortalidad en el mundo occidental. La disminución de la mortalidad de esta “plaga” se ha mantenido constante desde 1975 aunque con un ritmo inferior desde la década de los 901. Ninguna otra patología es tan prevalente o conlleva tantos gastos sanitarios, estimándose que a partir de los 60 años, entre el 55-79% de la población sufre algún evento cardiovascular2. A pesar de los avances diagnóstico-terapéuticos, una población cada vez mas envejecida, un mayor número de patologías asociadas y el aumento de los factores de riesgo cardiovascular, determinan que esta patología se mantenga como una importante causa de muerte.
Si se focaliza el problema en la población insuficiente renal, el riesgo de enfermedad cardiovascular, la morbilidad y la mortalidad permanecen fuertemente elevados para todos los estadios de la insuficiencia renal crónica (IRC) siendo especialmente alarmante en los pacientes com IRC terminal em tratamiento substitutivo3, 4.
Por otro lado, esta patologia “sistémica” aterosclerótica, es también responsable por la creciente prevalencia de la IRC, la otra nueva “plaga”. La enfermedad renovascular aterosclerótica (ERVA), ya descrita desde la década de los 605, 6, determina además de la conocida hipertensión vasculo-renal por estenosis arterial, otra patologia de extrema relevancia en el momento actual: la nefropatia isquémica (NI), responsable del 5-22% de las causas de IRC en los pacientes mayores de 50 años7. El término de NI no traduce sin embargo, una situación de hipoxia o verdadera isquemia sino la alteración de la función renal secundaria a la enfermedad oclusiva de las arterias renales.
Establecer la prevalencia de esta última patologia en la población general, no es posible dada la falta de métodos diagnósticos simples aplicables a gran escala. La mayoria de los estudios se han realizado en poblaciones de riesgo8, 9 (coronariopatia, enfermedad vascular periférica, insuficiencia cardíaca e HTA). Sin embargo es importante resaltar que no es una patología aislada; es un hallazgo frecuente en los pacientes con vasculopatia periférica, en los pacientes coronarios10, en pacientes ancianos con insuficiencia cardiaca11 y mas importante aún, que puede influenciar el pronóstico y la supervivencia de esos pacientes12. Es también un fuerte predictor de mortalidad a largo plazo. Además, no debemos olvidar que la NI supone una causa potencialmente reversible de IRC. Es decir, disponemos de medidas terapéuticas que pueden modificar el curso de la IRC, controlar la HTA y mejorar el pronóstico de los pacientes con cardiopatia isquémica y/o insuficiencia cardiaca asociada; no deberiamos pensar en ello?
Por otro lado, diagnosticar a tiempo esta patología puede resultar fundamental para evitar o disminuir otras complicaciones asociadas al uso de fármacos como los IECA o ARA II. Pilares fundamentales del arsenal terapéutico para los pacientes de riesgo cardiovascular, estos fármacos también han demostrado su beneficio en el tratamiento de la hipertensión vasculo-renal (uni y bilateral) y en la protección contra la progresión de la lesión renal13. La monitorización de la función renal en estos pacientes sería importante para obtener los beneficios terapéuticos sin crear nuevos riesgos (hiperpotasemia, insuficiencia renal aguda, edema agudo de pulmón..).
En la era de las nuevas tecnologias, son varias las alternativas disponibles para el diagnóstico de esta patología. Además de la arteriografia, considerada el “gold standard” otros métodos como la angio-resonancia, TAC helicoidal, Eco-Doppler, gammagrafia… han demostrado resultados satisfactorios. Sin embargo, la aplicación de estas técnicas no siempre es posible y condiciona un importante aumento del gasto sanitario si se utilizan de forma indiscriminada. El retorno a la medicina “Hipocrática” puede ser “sorprendentemente” una alternativa diagnóstica válida. Los resultados de las reglas o algoritmos de predicción clínica para la detección de la estenosis de la arteria renal pueden sobreponerse en sensibilidad y especificidad diagnóstica a la gammagrafía14, 15, seleccionando así pacientes de alto riesgo. Deberiamos pensar en ello.
La ERVA es una enfermedad progresiva9 (que puede llegar hasta la oclusión total) y potencialmente reversible como causa de IRC. La alteración del curso de la IRC con la modificación de los factores de riesgo, fundamentalmente con el control de la dislipemia ya ha sido descrita16, 17, 18, 19, 20, 21 y sería interesante disponer ya, de estudios prospectivos para valorar si el uso de las estatinas con beneficios descritos en la revascularización de las arterias coronarias22 podría ser útil en la estenosis aterosclerótica de la ateria renal. Por otro lado, ya están en curso ensayos clínicos para evaluar el impacto de estas terapéuticas en la morbi-mortalidad cardiovascular en los pacientes en hemodiálisis23, 24.
El tratamiento revascularizador con angioplastia percutánea mas stent25 y la cirugia26 ofrecen buenos resultados27. No obstante, a pesar de que este tratamiento puede controlar la HTA, preservar y recuperar cierto grado de función renal (incluso en pacientes en hemodiálisis) también puede resultar en fracaso28, 29 (manutención o agravamiento de la IRC, embolismos locales y/o sistémicos de colesterol…). La adecuada indicación y selección de los pacientes para un tipo u otro de revascularización así como la función renal basal, determinarán los resultados inmediatos y la evolución a medio y largo plazo.
A pesar de ser una patologia ampliamente conocida y estudiada, la nefropatia isquémica continua siendo una preocupación y un desafio para nefrólogos, cardiologos, radiólogos, cirujanos, internistas,… Mejorar el entendimiento de la fisiopatologia de la NI (interacción entre el estrés oxidativo, disfunción endotelial, la fibrogénesis y la lesión parenquimatosa…), una mayor intervención en los factores de riesgo y la definición de protocolos específicos con los criterios de elección para los diferentes tratamientos, permitirá abordar nuevas estrategias terapéuticas y podrá ayudar en nuestra práctica clínica. Tendremos que pensar en ello.

RENOVASCULAR DISEASE. ISCHEMIC NEPHROPATHY: PRESENT AND FUTURE.
In the last five decades, cardiovascular pathology (coronary disease, cerebro-vascular disease and peripheral artery disease) has been one of the principal causes of morbidity and mortality in developed countries. The decrease of this “plague” mortality has been continuous since 1975 although with a less ratio since the nineties1. No other pathology is so prevalent or has the same economic impact, being estimated that 55-79% of the population with more than 60 years-old, develop some cardiovascular event2. In spite of the advances in diagnosis and treatment, an ageing population, an increase of associated diseases and cardiovascular risk factors, are responsible for the maintenance of this pathology as an important cause of death.
If we focus the problem on renal insufficiency population, the risk for cardiovascular disease, morbidity and mortality continue strongly high for all groups of renal chronic failure (CRF) and it is especially alarming for patients with terminal chronic renal failure in dialysis3, 4.
In the other hand, this “systemic” atherosclerotic pathology is also responsible for the increasing prevalence of the CRF, the other new “plague”. The atherosclerotic renovascular disease (ARVD) described in the sixties5, 6, in addition to renovascular hypertension appointed another relevant actual pathology: Ischemic Nephropathy (IN) responsible for 5-22% of RCF causes in patients older than 50 years7. The term IN does not reflect a situation of hypoxia or real ischemia, but a renal dysfunction secondary to occlusive renal artery disease.
Establishing the prevalence of this last disease in general population is not possible due to the absence of simple diagnostic methods for a wide scale screening. Most of the studies have been done in risk population8, 9 (coronary disease, peripheral artery disease, heart failure and hypertension). However, it is important to emphasize that it is not an isolated disease; it is a frequent hazard in patients with peripheral artery disease, coronary disease10, in elders with cardiac failure11 and more important, it can influence the outcome and survival of these patients12. It is also a strong predictor of long time mortality. Besides, we should not forget that IN is a potential reversible cause of CRF. In conclusion, we have therapeutic options that can change the CRF course, control the hypertension and improve the outcome of patients with coronary disease and/or congestive heart failure; should we not think about them?
In the other hand, an early diagnostic of this pathology can be essential to avoid or decrease the complications associated to the use of drugs as angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers. These drugs that are the main stone of the treatment for cardiovascular risk people, have demonstrated a beneficial effect for the treatment of renovascular hypertension (uni and bilateral) and as protectors against renal progressive injury13. The surveillance of renal function in patients undergoing these drugs is important to get the therapeutic benefits without new risks (hyperpotassemy, acute renal failure, flush pulmonary edema…).
In the age of new technologies there are several alternatives for diagnosis of this disease. Besides of arteriography, considered as the “gold standard”, other methods as magnetic resonance, spiral TAC, Eco-Doppler, cintigraphy… have demonstrated satisfactory results. However, the application of these techniques is not always possible and is an important increase in health resources if they are used in an indiscriminate way. The return to “Hippocratic” medicine results in a surprising valid diagnostic method. The results of rules or algorisms for medical prediction in diagnosis of renal artery stenosis can be overlapped in sensibility and specify to cintigraphy14, 15, choosing then high risk patients. We should think about it.
The ARVD is a progressive disease9 (with eventual evolution to arterial occlusion) and potentially reversible cause of CRF. The change of CRF course with risk factors modification, specially dyslipemia have been described16, 17, 18, 19, 20, 21 and it would be interesting to have prospective studies to evaluate if statins, with beneficial effects in coronary artery revascularization22, would be useful in renovascular disease. Moreover, we have in curse clinic assays to evaluate the real impact of this treatment in cardiovascular morbidity and mortality of dialysis patients23, 24.
The revascularization with percutaneous angioplasty with stent placement25 and surgery26 has good results27. Nevertheless, this treatment that can control the hypertension, preserve and recover part of renal function (inclusive in dialysis patients) may result in a misfortune28, 29 (maintenance or deterioration of CRF, local or systemic cholesterol embolism…). A correct indication and selection of patients for one or another type of revascularization and basal renal function, will determinate the immediate results and the medium and long term outcomes.
The NI, a known and studied pathology continues to be a preoccupation and a challenge for nephrologists, cardiologists, radiologists, surgeons, and internists… Improve the knowledge of NI phisiopathology (interaction between oxidative stress, endotelial disfunction, fibrogenesis, parenquimal injury…), a better intervention on risk factors and the development of specific protocols with inclusion criteria for each treatment, will allow us to use new therapies and to help in our clinical practice. We will have to think about it.
